I am a student doing some research into how people choose therapists. If you are currently in talk therapy or if you have seen a therapist in the past, I would be very grateful for your help in completing my survey!
The survey should take only 5-10 minutes and you will be entered into a prize drawing for an 8-gig iPod Nano, engraved with your name or initials. Your responses will be completely anonymous and will not be attributed to you in any way. Please note that we will ask the winner of the iPod to confirm his or her responses in order to ensure accuracy.
I am also looking for 25 people who are willing to spare 15 minutes or so to chat about their experience choosing a therapist in a bit more detail over the phone. If you are interested in sharing your thoughts about this with me, simply enter your email address at the end of the survey, so that I can email you to arrange a time to chat. Your email address will not be used for any other purpose, or shared with any other party, and your comments will not be attributed to you in any way. In addition to being entered into the prize drawing for the iPod Nano, you would also receive a $25 gift certificate to spend on Amazon.com, as a token of thanks for participating in a phone interview.
