Thrombocytopenia and thrombosis

Recommended Posts

Could anybody help me to understand the relationship between thrombocytopenia and thrombosis. I was previously thinking that these are two opposite factors, but it turned out that these are cofactors (in vaccine-induced immune thrombotic thrombocytopenia and Heparin-induced thrombocytopenia). I read some articles about these but still cannot get it: how low platelet count may lead to thrombosis if thrombosis, by nature, is caused by aggregation of platelets in a vessel and high platelet count may cause thrombosis as well.
Thanks!

Wouldn't it depend on what's causing the platelet destruction.   If it were an autoimmune condition like lupus,  you could get vessels inflamed and clogged as the immune system turns on itself.  You'd have damage to vessel walls and platelet clumping -- ergo  possible thrombosis.   Heparin could do this,  too,  when antibodies are formed against it.   Kind of ironic,  yes,  because you have a blood thinner that's resulting in clumps that lead to blockages.

In some cases you can have a thrombus in some of vessels (previous clot), while you have a current low platelet count (thrombocytopenia).

A thrombus is coagulated platelets, which will not form in heparin induced thrombocytopenia.

Maybe you are thinking on IMHA (Immune Mediated Haemolytic Anaemia)?

In this case, the red blood cells attacked by the individuals own immune system.

There is another condition, DIC (Disseminated Intravascular Coagulation). In this case sepsis, surgery, trauma cause coagulation in smaller vessels all over the body, creating a lot of small thrombus, using up all factors of the coagulation cascade and so creating thrombocytopenia, what can be the reason for different types of bleedings.
