Could you please explain what it is you don't get and members here will address the specific issues. My advice would be to start your search with this website. To get you started, here's a paper I found by using the key words 'macrolides' and 'hepatotoxicity'.

I ask because I am a medical student trying to understand the mechanism by which Macrolides cause the side effects they do. When I say "detail" I don't mean an essay. I just want a step by step explanation. Thanks for the website link. Coincidentally, I used that exact link to search for my answer. I could not find anything solid that made me understand this topic. One journal article mentioned something about Macrolides damaging mitochondria within hepatocytes which causes oxidants to be released which damage hepatic ducts leading to build up of bile acids. But because this explanation is so superficial I know I will not be able to retain it. Hence I decided to register on a more specialist forum to get my answer. Thanks for the link to the journal article but I cannot access it.
